Main purpose of the Rehabilitation Case Manager job:

Responsible for customer delight, clinical excellence and achieving positive outcomes on cases, with a primary focus on suitable and sustainable return to work outcomes.

This Rehabilitation Case Manager role delivers case management services to agreed standards for a designated group of customers.

The Rehabilitation Case Manager is responsible for the planning, co-ordination and monitoring of an individual’s rehabilitation. You will work alongside vocational and occupational health consultants, medical practitioners and employers to facilitate the injured person’s recovery and return to safe, suitable and sustainable employment and a fulfilling life.

Location: Located in Bristol; travel to client / customer sites within the UK may be required, on occasion.

Reports to: Head of Health Services, with supervision and support from Aviva Health Clinical Team Leaders

Rehabilitation Case Manager – Selection and progression:

Our graduates undergo a robust development programme and receive ongoing training and support to progress through their role.

HCML have guidelines to determine the length of your graduate programme, however this is also dependent on your progress.

You will commence as a Rehabilitation Case Manager in a team that either manages minor musculoskeletal injuries or minor back and neck injuries to start. Case management is conducted telephonically with the emphasis on rehabilitation client education and self management. This provides an excellent introduction to the principles of case management.

Rehabilitation Case Manager – Key Accountabilities

There are team specific processes and procedures, however the main duties and principles of case management apply as described below:

• Customer contact and liaison:

Take account of customer requirements when delivering case management services; provide high standards of customer care

• Service delivery:

Work in line with agreed service standards, KPIs, timescales

• Quality (clinical and service excellence):

Deliver case management services to meet high standards of clinical decision making and achieve the best possible outcome for clients; seek advice from the Clinical Team Leaders or Interim Head of IMA Services where necessary; participate in file reviews and audits and comply with recommendations to improve the quality of service received by customers

• Working relationships:

Work as a member of the team to achieve the best outcomes for clients and the best service for all customers, demonstrating flexibility and team spirit

• Corporate responsibilities:

Work in accordance with policies and procedures in relation to Health & Safety, Equal Opportunities and Diversity, and other company policies

• Other:

Any other projects or tasks relevant to the team or department, as directed by the Interim Head of IMA Services

• Systems:

All case work is done through the case management database; also regular use of MS Office software, internet, other databases

Rehabilitation Case Manager – Person specification

• Experience:

Relevant background eg health / NHS, case management, insurance, vocational rehabilitation

• Qualification:

Relevant degree eg nursing, Occupational Therapy, Physiotherapy, Sports Rehabilitation (Exercise Physiology), Sports Therapy, Psychology, or some other appropriate profession; possibly HCPC membership; will be a member of Case Management Society UK

• Skills:

– Communication (face to face and by telephone), assertiveness
– Effective documentation and reporting skills
– Understanding of the medical recovery and rehabilitation process (rehabilitation code of practice)
– Clinical awareness in line with current evidence based practice and experience in either general orthopaedic, musculoskeletal, neurological and / or psychological injuries
– Setting goals and planning to meet them
– Written communication, including report writing, and disciplined approach in case noting
– Good clinical reasoning and problem solving skills
– Understanding of the private and public health systems
– Ability to understand and relate to commercial / industry requirements (key parties)
– Competent word processing and computer literacy skills (Word, Excel, general computer skills, email)

• Other:

Flexibility around working hours and ability to travel throughout the UK for occasional meetings
