Personal Advisors role is to befriend, advise, assist and support young people leaving the care of the local authority and moving onto independence as per the Children Leaving Care Act 2000. To review or support the review of the pathway plan, and when necessary amend previously agreed recommendations set out in the young persons Pathway Plan, for young people, who are currently or were formerly looked after by the local authority, their carers and support networks to achieve the recommendations in the plan. To work directly with care leavers, aged 16 plus, to facilitate their transition to adulthood and independence from local authority care, ensuring the young peoples objectives that are set out in their Pathway Plan are met. Undertake assessments such as Community Care Assessments to ensure asylum seeking young peoples entitlement to leaving care support is ongoing if Immigration status changes. Responsible for developing close and effective working relationships with key statutory and voluntary agencies and partners, such as Health and Education agencies, Department of Work and Pensions and UK Borders Agency, and to facilitate a planned and coordinated multi-agency response to the identified needs of care leavers. Liaise with other local authorities when a young person is residing outside of Surrey to ensure pathway plans are updated and reviewed in line with Surrey policy and procedures for leaving care services, in line with the National Inter-Authority Protocol for Care Leavers. To prepare and maintain appropriate written records in line with 16 plus Service recording policy and to update the authorities recording system, ICS, with significant changes with regard to Pathway Planning, subsequent transfer and case closure. To liase with various agencies in pathway planning to ensure young people are aware of their entitlements in regards to their asylum outcome. To take part in a duty system, answering calls from young people who’s assigned worker maybe unavailable. Familiarise, uphold and promote the aims of the council’s Equality and Diversity policies in the course of day-to-day work. Ability to develop and sustain good working relationships with young people including those who may be disengaged. Commitment to user involvement Ability to identify risk and seek guidance where necessary Ability to liaise and communicate effectively with key agencies Good organisational and administrative skills Ability to work as part of a team and on own initiative. Direct experience of involving users in service delivery/planning. ICT Skills Experience of working with young people in youth work or social care settings. Experience of working with unaccompanied Asylum Seeking young people
