Senior Supported Housing Worker

A Senior Supported Housing Worker vacancy has become available in Hounslow.

You will organise and plan effective housing support and housing management interventions to service users at the hostel. you will have lead responsibility for the housing management, well being and support planning for service users. you will carry out all duties in line with organisational policies and procedures.

KEY RESPONSIBILITIES:

To be responsible for the support and well-being of all service users, using person-centred planning a key work system to enable them to attain and sustain a tenancy, enjoy a good quality of life and avoid repeat homelessness.

To assess referrals, formulate and review support plans, risk management plans and move-on plans with all service users.

To allocate key-workers, coordinate and supervise support assistants to ensure that support plans, risk management plans and move-on plans are effectively implemented.

To work in partnership with outside agencies involved in the care and support of service users.
