Extra Care Coordinator

My client is looking for an Extra Care Co-ordinator to join their busy service supporting elderly clients in supported living

I require a committed and enthusiastic individual to work alongside the Registered Manager to lead the coordination and management of a very high quality Extra Care service in Slough. You will have the values and commitment to lead the staff team to provide the highest standards of customer care and person-centred service delivery and to promote independence, choice and wellbeing.

The role will involve direct care, staff deployment and supervision of a highly motivated team of Extra Care workers.. You will be expected to work effectively with families and other agencies and to maintain a positive and trusting relationship with the on-site Housing Scheme Manager. This is a busy service and therefore you will need to be hard working, accountable and have strong organisational skills.

This is an exciting opportunity to work in partnership with other agencies to provide a holistic and joined up approach for meeting the needs of older adults. Working within one Extra Care service you will be able to develop positive professional relationships with individuals and offer support to people both in their own flats and in the well-resourced communal facilities.

You will have at least two years relevant experience gained in the care and support of older people in Extra Care, home or residential care. You will be expected to bring a warm, caring and person-centred approach to your work and to be able to work collaboratively with our service users and their families. You will demonstrate a commitment to reablement principles and to promoting dignity and independence. You will need excellent communication skills with good attention to detail and the ability to follow up issues and complete tasks.

A health or social qualifications is required, such as NVQ 3/4/5. In addition you will have had a good general standard of education. In return we can offer you good conditions of service and personal development opportunities. This is a part time role, 30 hours per week.
