Clinical Case Manager

Salary: £36,000 – £42,000 per year

Must be HCPC or NMC Registered to apply

Occupational Therapist, Physiotherapist or General Nurse Home Based £30-35k basic + Car Allowance + Working from home Allowance.

My client is the leading provider of Case Management services in the UK and they are seeking a self-directed, autonomous practitioner to join their UK wide team. You will case manage a group of 30+ patients who are based throughout your territory. The territory you cover can be up to 2 hours from your home.

In order to be successful in this role, you will need to have a minimum of recent two years trauma orthopaedics.

Day to Day

* You will initially assess and then manage the case of a wide range of patients. Typically, you will work with a case over 18 months. The patient groups will have complex orthopaedic injuries, spinal injuries and acquired brain injury.

* You will be expected to manage your caseload remotely and when required reassess the patient. Lead the case management for individuals who have sustained complex physical or psychological injuries.

* Assess the individual’s needs to develop and manage a treatment plan and overall return to function and work.

* Work closely with the individual and their employer to achieve positive outcomes.

* Write findings and recommendations in reports to stakeholders.

* Management of a financial delegation per case.

* As the position is field-based, there will be a requirement for travel and the occasional overnight stay.

Requirements

A background in case management or orthopaedics is crucial. Interest in case management and demonstrating you can commit to the role and the patient group.

Training

You will receive an intensive training course which typically lasts 6 months. You will have 1-2-1 support from your manager and the opportunity to develop further within the role.
