About the role
This is an exciting opportunity to be part of a brand new Domiciliary Care company! Heritage Healthcare Greenwich are recruiting an experienced Care Coordinator to join our expanding team and work alongside the Registered Care Manager.

As a Heritage Healthcare Care Coordinator, you will be responsible for ensuring that high standards are maintained throughout the company at all times and have excellent communication and leadership skills.

You will also provide support services and personal care which will help to make a real difference to the lives of our clients. You will build relationships and often friendships with our clients and their loved ones– you will ensure they receive great quality care which is delivered from your heart.

Own transport is essential and applicants must be flexible as hours will include days, evenings, weekends and nights. I.T. experience is essential.

Duties will include:
•Initial Service User Assessment
•Reviews and Monitoring
•Managing Staff Rotas
•On-Call Support

We are looking for:
•Qualifications: You will have completed your NVQ Level 3 in Health & Social Care
•Experience: You will hold substantial experience at supervisory level and in a care setting
•Personal qualities: Enthusiastic, caring, friendly, trustworthy, compassionate and reliable
•Transport: You will hold a driving licence and have access to your own vehicle
•Flexibility: Your hours will include days, evenings, weekends and nights
•Computer literate: IT experience is essential and previous use of care software would be an advantage
