JOB PURPOSE: To work as Team Leader of the team in the provision of care and support services for people with mental illness, learning disabilities, autism and/or behaviours that may challenge; which aims to maximise the potential of individuals in line with ordinary life principles.

JOB DESCRIPTION – TEAM LEADER.

Job Title: Team Leader for supported living scheme’s

Reports To: Branch Manager.

Purpose of position

To lead the team of supported living support workers and ensure the smooth running of the supported living schemes you are responsible for.

2. To be on call for the whole of your area and give support as and when required.

Principle responsibilities

1. To assume all the duties of a Support Worker when not engaged on Team Leader duties

2. To ensure support workers deliver all their duties as per their job description

3. To coach and support the support workers to achieve item 2 above

4. To ensure all elements of each senior and support workers job descriptions are delivered

5. To ensure best practice in all service delivery across all areas of responsibility

6. To assume delegated responsibility for accounting for all monies spent on behalf of service users, and to produce receipts for any expenditure, including responsibility for the actions of a designated staff team (support worker) in this respect, on a weekly basis.

7. To ensure weekly safety and maintenance checks are carried out and the correct action taken

8. To ensure medications are checked weekly, that all meds are signed for where administered and that the PRN stock is checked and reordered when required.

9. To ensure all key workers complete monthly reports and that they are delivered to the office together with monthly dairy reports.

10. To check communication book/dairy on weekly basis to ensure that these books are being used correctly.

11. To participate in taking a delegated lead role in the development of programmes based upon individual needs, and to be involved in putting these into practice.

12. To work with service users, both on an individual and group basis, on a wide range of activities which meet identified need and which contribute to securing a valued life-style for the person/people involved.

13. To support service users, and ensure support for service users, in all aspects of personal care, as appropriate.

14. To facilitate and enable service users to access and utilise the resources and activities available within the wider community.

15. To contribute to individual reviews and plans, as required.

16. To liaise regularly with service users’ family members, friends, advocates and any professionals, as necessary.

17. You may on occasions be asked to escort a service user to and from activities using your own vehicle where this is the case you must ensure that you have class 1 insurance to cover all business activities.

18. To ensure that all Service User are Safeguarded in accordance with local protocols at all times.

19. To carry out any other reasonable duties which are within the scope of this post.

20. Ensure appropriate staffing levels are maintained to provide effective care provision.

21. Manage staff sickness levels, organising necessary cover on a day to day basis.

22. Familiarize the team with clients needs and ensure the team address all relevant areas of care within their individual sphere of employment.

23. Organise & facilitate regular staff meetings.

24. Provide necessary organisational information to staff team.

25. Help keep the team focused & on track on Personalisation and within PCP

26. Attend regular meetings as directed by management.

27. Ensure maintenance of house environment is of a high standard (PAT testing, light bulbs, décor, general repairs).

28. Keep the management team informed of accomplishments & on-going issues, participate in achieving resolutions to identified issues.
