Salary: £26,000 per year

Role: Independent Living Support Worker

Location: York & Market Weighton

Salary: £26,000per year, or £26,000 per year if you have an NVQ 2 or above in Health & Social Care.

Supporting individuals to increase or maintain their independence by enabling them to choose opportunities which enhance their quality of life.

Providing a high standard of person-centred care working in line with companies values and behaviours and the essential standards as detailed by the Care Quality Commission.

Main Responsibilities:

* To build trusting relationships by getting to know the individual, understand what is important to them and how they want to be supported within agreed company procedures

* To communicate effectively with individuals in ways they understand so they can make informed choices and decisions about what is important to them.

* To involve individuals in the design, development and delivery of their service

* To support and encourage individuals to be part of their community, with social, educational and daily domestic activities of their choice. This will include occasionally driving individuals in their own vehicles to places of their choosing or accompanying individuals on public transport

* To ensure individuals receive assistance appropriate to their needs, including personal care, help with finances and managing their home, medicines and other aspects within their agreed support plan

* To record and monitor the support given by writing and updating care records accurately, within agreed standards of record keeping.

* To work proactively with other members of the ILS team to provide a safe living environment for the individual

* To maintain good communication links with significant people to the individual including friends, family members and health and care professionals where appropriate.

* To attend training and team meetings to be a full member of the team and contribute to service development.

* To ensure the service is compliant with health and safety and care requirements to undertake checks as directed by the service manager.

* To administering medications to individual residents, where this is part of the care and support plan, following the company medicines policies, procedures once deemed competent to do so following training
