The Social Workers in this team will have responsibility for advising and developing social care support, assessing eligibility and researching services available in the local community. Inputting appropriate referrals onto FWi, for carrying out urgent face to face and at times telephone assessments, for making a decision about eligibility and support services required by customers and setting up appropriate support plans or establishing achievable goals, for liaising with internal & external services regarding welfare checks for customers following prevention of admission and problem solving any issues relating to social care intervention, for referring on to the appropriate back end social care team and for appropriate case recording of all of the above on FWi. This role is therefore crucial as we move towards a model of health and social care integration. The Hospital team is a fast paced and responsive team it is important that it has sufficient social care assessment capacity to meet the demands and the time scales working across Health & Social care. The use of different services such as housing, preventative services; i.e. Substance Misuse will help to develop skills by redirecting to appropriate community resources. The response time will be in line with statutory requirements of Sct2 two days & Sct5 24hours under the Hospital discharge Act 2003
The post- holder will be working within the Transitions Team in Support Planning and Review and will be responsible for assessments and care planning for children, young people with disabilities

Purpose of Job:
To carry out needs led assessments of children, young people and young adults within the guidelines of Children’s Act 1989 and 2004 and Community Care Act.
To care manage straight forward and complex cases including Safeguarding Children, Young People and Adults and Deprivation of Liberty and mental capacity. Co-ordinating and reviewing multi-service care packages.
To support the unit in meeting individual / Unit performance targets and actively contributing to its continuous improvement.
Principal Accountabilities
To carry out Needs- Led assessments of children and young people with disabilities and their carers in accordance with the council’s policy and children’s statutory framework.
To formulate appropriate care plans and negotiate care packages with service users and service providers. This will involve formulating multi-service care packages, and reviewing on a regular basis.
To carry out all duties in accordance with the council’s policy and procedures.
To carry out needs led assessments, support planning and arrange services for children and young adults with disabilities within statutory requirements and Council Policy
To apply eligibility criteria to determine eligibility for support and other children’s statutory eligibility criteria as appropriate.
To identify appropriate services to meet assessed eligible need
To provide support plans to service users in receipt of services from the department.
To regularly monitor and carry out within guidelines of the Community Care Act and Children’s Act statutory framework timely 6 weeks/12months reviews of services provided to [adult service users] and statutory review timescales for children’s service users within allocated caseload. This will include reviewing multi-service care packages.
To update care plans following reviews, and negotiate care packages with service providers.
To be familiar with Brent’s policies and procedures and appropriate legislation procedures and resources.
To maintain appropriate and accurate up to date service user’s records and to provide statistical information and reports as and when required. To co-operate in research projects where appropriate
To contribute to the development, planning and implementation of policy and practice relating to the care management of service users and carers.
To facilitate and foster effective relationships between local authority, NHS, private and sector partners and service providers and to represent the service at meetings or relevant panels, chairing and minuting when required, in order to facilitate the effective provision of services to users.
To prepare young disabled people for transition to adulthood.
To maintain Social Worker Registration through the General Social Care Council, and observe the professional code of conduct at all times.
Enhanced CRB check
To have responsibility for a portfolio of cases for a defined area of work as determined by the line manager
Participate in the rota duty system at all times with other members of the service unit and to respond to Emergency situations when required.
