Social Worker – Adults

Social Worker required to assess the needs of service users over the age of 18 in a wide range of settings (hospital, community, residential homes etc) The social worker must have a good understanding of the Care Act in relation to eligibility criteria. The social worker would also be expected to complete support plans and commission services to meet the needs of the service user and review these needs. The social worker needs to be aware of the Mental Capacity Act and Continuing Health Care funding streams. car driver essential in this role.

You must hold a recognised professional Social Work qualification (MA/BA/BSc in Social Work, Diploma in Social Work or CQSW) and be HCPC registered. Additionally, you must have at least two years post qualifying experience and be able to provide five years of references.
