Job Title: Social Prescribing Link Worker

Hours: Full-time (35 hours)

Type: Fixed-Term (until March)

Salary: £27,000 per annum

Location: Remote with some visits

About us: Community Connex is a campaigning organisation and a multi-project service provider working across West London, supporting disabled people, those experiencing other disadvantages and their families to live healthier, happier, and inclusive lives by ensuring their needs, rights and aspirations are met.

Key duties:

• Promote social prescribing support and deliver one-to-one appointments to people with a range of social needs.
• As part of the PCN multi-disciplinary team, build relationships with staff in GP practices and other health and social care professionals.
• Provide personalised support to individuals, their families and carers to take control of their health and wellbeing, live independently and improve their health outcomes.
• Support people to overcome their personal barriers to access voluntary, community or social enterprise organisations.

Our ideal candidate:

• Ability to actively listen, empathise with people and provide person-centred support in a non-judgemental way.
• Able to get along with people from all backgrounds and communities, respecting lifestyles, and diversity.
• NVQ Level 3, Advanced level or equivalent qualifications or working towards or professional experience.
• Experience of supporting people impacted by socio-economic worries, enhance employability, and improve metal wellbeing.
• Experience of supporting people, their families and carers in a related role (including unpaid work).
• Knowledge of the personalised care approach.
