£29,000-£31,000 per annum

Due to the growth of the business, we have an exciting opportunity for a motivated and personable individual to manage a team of Care Liaison Advisors responsible for sourcing packages of care for patients with a variety of needs. Your team will play a vital role in ensuring a safe and quick discharge in line with patient’s care plans, whilst supporting families often during a great time of stress. This is a high-quality service that reduces unnecessary delays in the discharge process.
As a Scheme Manager, you will be responsible for monitoring the teams activity to ensure referrals are proactively progressed and processes are followed, monitoring their KPIs and providing updates to key hospital contacts and commissioners. We’re looking for a strong problem-solver that will act as a point of escalation for the team and our clients, to ensure a world-class customer service is delivered at all times.

Every day you will…
- Manage a team of Care Liaison Advisors by conducting team meetings, 1:1s, delegating workload and monitoring productivity and KPIs to ensure referrals are proactively progressed and processes are followed
- Act as the main point of contact for hospital colleagues and commissioners, working closely with the hospital discharge team and acting as an ambassador for CHS
- Build relationships with healthcare system colleagues and gain an understanding of the wider teams, systems and processes and support the Operations Manager in seeking out opportunities to provide further support or services
- Lead monthly contract review meetings, reporting and presenting on KPIs and data outcomes

To thrive in this role you must ideally have…
- Experience in managing a team
- Healthcare experience
- Excellent written and verbal communication skills with experience in a customer facing role
- A full UK driving license and access to your own vehicle
