Female Mental Health Transition Support Workers – Goq

Our client is looking for mature female Transition Support Workers who have at least 12 months experience of working with adults with mental health concerns, learning disabilities and behaviour that challenges.

The role involves working with a team of staff supporting service users through the transition period from secure hospitals and units into a supported living environment. Your responsibility will be to build rapport, and develop a relationship with an individual enabling you to understand their needs, requirements to be able to support them during this period.

Successful candidates must commit to 37 hours a week working a mixture of AM’s, PM’s Waking Nights or Sleeps. This role requires candidates to undergo an Enhanced DBS check and provide two years worth of referencing.
