Do you have a degree in nutrition or sports science and are looking for an exciting opportunity to develop your skills and make a life changing impact to people’s lives on a daily basis?

We are looking for a passionate and knowledgeable individual to join us as a Diabetes Practitioner, this will be to support the delivery for both the digital and face to face T2DR (Type 2 Diabetes Pathway to Remission) Programme. You will be focused, and results driven with patient interaction being at the heart of everything you do demonstrating an empathetic and motivational approach to help them achieve their goals! Helping and encouraging people overcome potential barriers on their journey will play a key part in your role.

Every day you will be:

  • Delivering 1:1 sessions ensuring fidelity to the programme curriculum content to enable and encourage service users to change their behaviour and improve overall health
  • Conducting progress reviews with service users over the telephone, ensuring patients are progressing through the programme and answer questions patients may have
  • Co-ordinating appointments and supporting the team of Patient Coordinators with referrals and bookings when required and providing information on the programme when required
  • Conducting assessments with the service user, taking clinical and non-clinical measurements including validated questionnaires, height and weight, blood pressure, blood glucose levels
  • Liaising with Service Users, Management and Senior Management teams, and key stakeholders whilst being in attendance of regular meetings with the local delivery and operational team

To thrive in this role you must have:-

  • Minimum of undergrad Degree in Nutrition or Health Related Science (Sports/Exercise Science/Health Psychology)
  • Experience and ability to demonstrate being able to tailor curricular contents to the needs of individuals or groups with excellent communication skills to deliver effectively
  • Previous experience in 1 to 1 and group facilitation and teaching, delivering information tailored to adults in a confident, engaging and interactive manner
  • Driving license with access to your own vehicle as you will be transporting programme delivery equipment and expected to travel in your local area in order to deliver the sessions
