We are currently seeking dedicated individuals to join our hospital discharge team. Rate of pay – £30 to £35 per hour

Location – Cheshire West

Responsibilities of this role:

  • Conduct comprehensive assessments of adult patients’ physical, emotional, and social needs to develop a deep understanding of their individual circumstances.
  • Coordinate and facilitate safe and timely discharge plans
  • Provide advocacy and support to patients and their families
  • Conduct risk assessments and implement safeguarding measures to ensure the well-being and safety of vulnerable adults during the discharge process.
  • Collaborate closely with multi-disciplinary teams, including doctors, nurses, therapists, and community services, to ensure a holistic approach to patient care and effective coordination of services.
  • Maintain accurate and detailed documentation of assessments, care plans, interventions, and communication with patients and other stakeholders.
  • Assess and address any practical, financial, or housing-related needs that may impact the patient’s successful transition to community care.

Requirements of the role:

  • Social Work England registered
  • 2 years experience in similar role
  • Must hold a UK driving license
