You will be working closely with Hospital Staff, helping and supporting adults ensure the patient receives the correct healthcare plan and maintenance even after being discharged from the hospital.

We are currently looking for an experienced and dedicated Social Worker to work full-time within the team. The ideal candidate will possess highly developed specialist knowledge of social work practice in Adult’s service.

Duties and Responsibilities:

Will be working closely with hospital staff.
Undertake assessments and help service user plan
Work alongside multi agency services consisting of nurses, Doctors and support workersRequirements of the Social Worker:

Hold a degree, post graduate or Professional Qualification in Social Work
Full working driving license
