Adult Social Worker – Transitions / Preparing for Adulthood Team

Social worker needed for the Preparing for Adulthood Team within the Integrated Learning Disabilities Team. Candidates must have experience of working with learning disabled people and of completing Care Act assessments. The role works with young people with learning disabilities age 17-25 through transition from Children’s to Adults services.
