About us: Community ConneX is a campaigning organisation and a multi-project service provider working across West London, supporting disabled people, those experiencing other disadvantages and their families to live healthier, happier, and inclusive lives by ensuring their needs, rights and aspirations are met.
Key duties:
- Implementation of quality control procedures.
- Conduct client assessments prior to commencement of care service and following up with relevant team members to ensure that the information provided are utilised for effective service delivery.
- Attend client’s first call to introduce the care worker to the client, facilitate orientation and good rapport between staff, the service users/clients, and their significant others/families.
- Liaise closely with Registered Manager to ensure correct client/care worker matching for new clients.
- Work flexibly and in emergency circumstances to cover care calls to clients who otherwise would not receive a visit.
Our ideal candidate:
- Qualification in Health & Social Care – Minimum of NVQ level 2 or 3.
- Excellent communications and writing skills, and the ability to tailor written and verbal communications to a wide range of different audiences.
- Relationship Building – maintaining existing and developing new relationships. Handling more complex relationships e.g., social workers, GP’s etc.
- Passionate about supporting vulnerable people.
