Salary: £36 per hour

Qualified Social Worker – Adult Intermediate Care – South London – £36 P/h

3D Recruit have worked with this south London local authority for several years and have developed a strong relationship with them. Our client needs to appoint multiple Qualified Social Workers to join their Adults Intermediate Care Team. As Qualified Social Workers you will be working in the new ‘@Home’ service, on a full time, 3-month contract which will likely be extended to a year.

About the Job:

* As part of this new service, you will be an integral part of the team when responding to those in crisis

* The team is comprised of Qualified Social Workers, GPs, Pharmacy, Therapists and Nurses, so a multi-disciplinary approach is required.

* You will be required to provide urgent response, admission avoidance and reablement for residents.

* Experience of learning disabilities would be advantageous.

* You should be able to competently carry a complex caseload.

* You will be conducting face to face visits (PPE provided).

* You must be able to demonstrate your knowledge of the Care Act, Mental Capacity Act, safeguarding and other complex requirements that occur when dealing with service users and their families.

* Problem solving is a key skill required for the job

* Home working will be considered

Requirements for a Qualified Social Worker working in the Intermediate Care:

* Social Work England Registration.

* Social Work qualification.

* Experience working with adults and older people with a physical disability.

* Experience of hospital discharge, community neuro or multidisciplinary team working.

* DBS on the Update Service or happy to apply for one.

* Right to Work in the UK.

* UK experience (preferred)
