Community Psychiatric Nurse

Salary: £30 – £31.2 per hour

ENS Recruitment have a 12 week placement for an experienced Community Psychiatric Nurse in Grays.

You will be responsible for the assessment of care needs and the development, implementation and evaluation of programmes of care for service users who are referred to the recovery and wellbeing team for Assessment/Review or Treatment.

You will be a member of a multi-disciplinary team and work in close co-operation with hospital based and primary health care based professionals. In addition, you will also be responsible for ensuring that an integrated assessment and treatment service is provided to the clients and ensure that a good inter-disciplinary working environment is maintained.

You will also hold responsibility for assessing, creating and reviewing social care packages and maintaining key performance indicators in health and social care.

You will be required to work with older adults, with a mental health diagnosis including Alzheimers, dementia and cognitive impairment. As integrated team skills would include completing care and support assessment, facilitating discharge of patients from hospital.

Having good IT skills is essential along with the ability to communicate effectively and to work within a community setting,with a degree of flexibility, organisational skills and a willingness to work as part of a team.

Hours will be Monday-Friday 09:00am-17:00pm
