Midland Heart is working with Lancashire Care Foundation Trust to deliver a home from hospital service within Royal Preston Hospital to support people to regain their independence after an illness or injury.
We are looking for highly motivated individuals who are committed to the delivery of customer centred services. You will need to demonstrate excellent inter-personal skills, effective communication, innovative solutions to problems and team working
Part of Midland Heart’s mission is to “create opportunities for individuals to realise their full potential”. Supporting people to have as much independence and control in their life runs throughout everything we do at Midland Heart – regardless of people’s age, circumstance or nature of their disability or ill health.
Beechwood provides 24 places for people aged 18 and over who are being discharged from hospital but for a variety of reasons are unable to return home in the short term.
You will be part of a team that provides our customers with a personalised service of reablement which supports customers to regain and maintain an independent lifestyle after an illness or injury which may include personal care and support planning.
We are now recruiting Reablement Workers to join the Health & Disability team at Beechwood. You will have an understanding of the importance of enabling and supporting customers to promote independent lifestyle after an illness or injury.
The successful candidate will have a passion for delivering high quality care. We are looking for self motivated individuals who are able to work as part of team and willing to attend training.
