About the role:

Band 5 Community / District Nurses. The post holder will be an autonomous practitioner who will carry continuing responsibility for a defined caseload of service users within the community.

Immediate start
ongoing contract
Full time – 5 days / week with weekend covers on rotation
Part time – 2-3 days / week

Requirements:

Full UK driving licence and access to own vehicle

At least 6 months recent community experience

Excellent communication skills, empathy, team player.
Responsibilities:

To undertake assessments of individuals and their families in the designated care group
The post holder will take responsibility for the effective assessment, planning and delivery of skilled nursing care to patients in their own homes or other settings as appropriate under the direction of the caseload manager.
Although the post holder has access to advice he/she is expected to work largely unsupervised. In addition, the post-holder is expected to supervise junior staff, Assistant Practitioner and Health Care Assistants including student nurses on placement.
