Qualified Social Worker – Adults Team – North Yorkshire

My client, based in North Yorkshire are looking for an experienced Qualified Social Worker.

The role is to start ASAP, working Monday to Friday 0900 – 1700.

This role will include:

• Completing contact assessments to FACS criteria
• Working with service users with complex needs including Mental Health & Disabilities
• Referring to appropriate teams or agencies-statutory and private sector
• Triaging and prioritising all adult referrals countywide
• Dealing with initial assessments, undertaking reviews. Undertaking long term assessments and taking cases to a validation panel for approval of funding.
• Liaising between agencies and other local authorities
• Liaising on behalf of service users and their families
• Establishing level of vulnerability
• Arranging changes in care plan
• Providing advice and information

Requirements:
You must have HCPC registration with the relevant qualifications and experience; and be eligible to work in the UK.
